Why was my Form W-9 rejected?

A Form W-9 may be rejected because the information provided is inaccurate or incomplete or does not match the details provided on the application form. When a Form W-9 is rejected, a notification is sent to the requester outlining the reason(s) for rejection. You need to submit a new request with the relevant details or/and requested documentation.
